Rapid Response Care / Emergency Care

Rapid Response Care or Emergency Care is a form of community care that is available at short notice and often in response to emergencies. We have been providing out-of-hours rapid response care services for more than four years in Pembrokeshire. The aim of the service is to prevent un-necessary hospital admissions and ease pressure points during the year (such as winter bed blocking), enabling individuals to be cared for efficiently at home instead of being admitted to hospital.

We provide a team of specially trained emergency care workers who are strategically placed around the county. The care workers are on standby and will respond to an individual within an hour of call-out to provide out-of-hours emergency care in the home. Referrals are placed by A&E departments, out-of-hours GPs, district nursing services, NHS Direct, walk-in centres and the social services emergency duty team.

A rapid response call-out will be followed up with a complete re-assessment of client needs with care programmes adjusted accordingly.
